What does this means:

>q3noclient.exe server ip port

- from XXXXXXX:XXXX
- to 0.0.109.56:27960

Error: Permission denied

Server port is 27960 so i didnt felt to use options. Even using -b it gives permission denied.

for sure one problem is in the parameters you have passed because I doubt 0.0.109.56 is the IP you specified:

q3noclient -s 27960 -c 27960 SERVER_IP CLIENT_IP

while the "permission denied" can be caused by the service pack you have... long story.
in short windows blocks spoofing so you need to use linux, change service pack or using a virtual machine

this depends by various factors, maybe you can send spoofed packets from your machine but you are behind a NAT or a router which drops your packet.
this is not a matter of works or doesn't work, it works if the packet reaches the destination with the correct parameters (source and destination IP/port)... otherwise is logical that doesn't work 8-)

Well neither using router nor NAT. Directly connected to modem and using DSL to be specific.

in this case check if your spoofed IP reaches the destination (ask this to a friend who can receive and see UDP packets, setting his IP as client because by default q3noclient sends the packet to the client) check that the port used by your client is exactly the same specified in the packet.
you can also try to invert the client and server parameters because, if I remember well, the thing works in both the direction
